All US participants must submit a BSA Annual Health and Medical Records form that is based on a medical examination dated after August 1, 2016:


All participants must use the the custom medical form for the jamboree.  The medical form consists of four (4) pages (parts A, B1, B2, C), plus a one-page risk advisory, plus a photocopy of the front and back of the medical insurance card.  All blanks must be filled in.  Write “None” where there is none, or “N/A” where it is not applicable.

PART A:  Scout must sign & date.  Parent/guardian must sign & date.  List parent/guardian as authorized adult(s).

(1)    Keep the original copy for your records.


(2)    Crew 158 copy:  Mail a paper copy by March 15, 2017 to:

        If you prefer (though the BSA does not recommend sending this information via e-mail)

        you may e-mail the Crew 158 copy of your medical form to:  stover@wsj207.us

        This deadline is necessary so that we can review the medical forms in a timely manner.


(3)    BSA copy:  If you will be attending the BSA National Scout Jamboree, then
         you must submit online by
April 15, 2015 using the following instructions:

        Please do not postpone the online submission to the BSA until the last moment because their online
        submission system has been rather problematic at times.
